Perhaps an interesting feature to the challenge could be immunity to disarm when using fist weapons but idk how that would affect PvP.
A bunch of fist weapons that are just bandages that overlay over the players' glove models would be a decent way to add variety to the fist weapon class and give low-level access to a weapon that isn't particularly intimidating but is still cool enough in it's own respect.
Would also be a cool feature to add in NPCs with quests at semi-regular intervals that reward fist weapons but perhaps place them in relatively out-of-the-way locations and players who pick the challenge spawn in with an extra item that is a book which gives hints of where to find the masters of the fist. Other players can still do the quests if they find them but the challengers get the hints to make it easier to know where to go to find fist weapons if they are falling behind in damage.
NPCs that come to mind would be a Friar in Westfall who discusses the art of pugilism, a Tauren Runemaster in the Barrens who discusses their niche style, a Night Elf in Ashenvale who has studied the agility of the Moonsaber, a Pandaren somewhere who discusses Drunken Fist style, a Goblin boxer in Booty Bay/Gadgetzan, Farakki wrestlers who perhaps have a quest to go to the arena area of Zul'farrak and summon an optional boss who does a lot of knockdowns, disarms, and interrupts, Dwarven strongmen flexing in the Badlands, a Nelf extolling the revitalising properties of hot springs in Winterspring, capoeira trolls dancing on the beaches of Stranglethorn, an undead apothecary in a graveyard doing some... bodybuilding..., an Orcish Shaman who provides a lightning enchantment that only affects fist weapons, a Witch Doctor brewing stamina potions which increase fist and unarmed damage for a short period of time, a Gnomish engineer who has developed rocket gloves...
There's a lot of flavour to be seeded out amongst the various races and around the world.
